The Person Centred Care Toolkit: From principles to practice
AGPAL & QIP Education and Training was commissioned by the Brisbane South Primary Health Network (BSPHN) to support their primary care workforce development to effectively implement the person centred care model within their region. The training, known as 'Person Centred Care Toolkit: From principles to practice' will support practice improvement using the Patient Centred Medical Home (PCMH) model and the key change concepts to facilitate incremental transformation to becoming a higher performing person centred care practice.
The training suite will include 10 e-Learning modules which are interactive and engaging. Each module features a range of activities, tools and resources to support translation of learning into practice.
